You don't "need" 5th. Just like a 4T65 doesn't "need" a good 4th clutch hub. Need is relative as long as the other gears seem ok. I figure between the clutch and 5th.. you want that out and fixed or swapped.

Having the trans rebuilt is a costly venture...last I knew it was about $2700 to get it done.
