Running lean in one bank?

Ok so I got my car back today and its running lean in bank 2. What causes that, bad fuel injectors or somthin else?

It's just a vacuum leak in the injectors?

Think it was something like 710. Don't think I kept the printout autozone gave me LOL. I look or scan again

Ok, I checked the injectors & replaced missing & broken plastic rings & also ran some fuel injector cleaner, to see if that would fix the problem. The SES light is still on & there's still a vacuum leak. I'm confused. Could it be a MAF problem?
