Sometimes the aftermarket 02 may throw a code once or twice. I usually check it with the scanner and if its working, i clear the code. If its really dead, it may sit and stay at 430some multivolts. Usually the code doesn't come back up after checking it and clearing.

The fan control relay circuit is most likely a relay going out or possibly rewired or spliced into.
