Multiple Codes - Help Needed. : Images attached

I just noticed yesterday while driving on the highway that my GTP was sputtering... I could feel it and see the rpm's bounce up & down.

Mods: 00 GTP - CAI - Headers - 3.4 Pulley - Autolite 606's

Hooked up HPT and went for a ride. The following 3 codes are active...

P0128 - Coolant Thermostat
P0140 - HO2S Circuit Insufficient Activity Bank 1 Sensor 2
P0171 - Fuel Trim System Lean Bank 1

I also noticed my LTFT is @ 16.4 which I read is indicitive of a vac leak?!

Also, There are a bunch of DTC tests pending, but never complete...

Any ideas?

Are you running an EGR?

If not go and set all the EGR codes to NO REPORT and those will never come up again.

The causes for a P0140 code are fairly few. They could be any of the following:

  • Shorted heater circuit in O2 sensor. (Usually requires replacement of heater circuit fuse in fuse block also)
  • Shorted signal circuit in O2 sensor
  • Melting of harness connector or wiring due to contact with exhaust system
  • Water intrusion in harness connector or PCM connector
  • Bad PCM

Sounds like the O2 sensor wires got shorted or the sensor died. That can also cause a lean code to pop up as the sensor is faulty and not giving proper readings.

That happened exactly. The Bank 1 o2 wires were against the headers and melted to one another. I got a new harness and o2. Just a week ago...
And no EGR. I couldn't find "no report" I was only able to turn off SES light :/
 
Well, guess I missed a spot when I sprayed the first time around. There is actually a whistling leak at the lim coolant bypass cover :/
