You can either buy the replacement panels they sell and cut out the rust and patch yours in or make your own panels to weld into place.
Either way you must cut the rust out, clean everything and then weld the new metal into place.

I didn't weld mine on my 97. I cut out the rust and riveted them in. I used bed liner spray on the inside and outside of the rockers and it held really well. Just had to touch up the back parts near the rear tires.

That's an extreme case but a very good thread with pictures of how to properly do it!
