P0455 code with values

I have a code on my car that's been on there for quite awhile. I changed the gas cap and it went out for 2 days and came back. I cleaned the plastic thing beside the filler neck lol and checked for cracks in the rubber line, all that looked good. Not sure if putting on dual magnaflow mufflers had anything to do w it. I show two codes for the same reason one says freeze frame. I have a lot of values along with this code but no clue what it means.

thats a large leak. under your car where the gas filler neck is, get under the car look up, you'll see the fat gas filler tube and another smaller line with it, the small one likes to rust out. its the tank vent tube. see if its wet with gas after a fill up too.

i had the same code forever, i got a tuner and deleted it, so i never gave a crap no more. till i smelt gas after a fill up and found this tube rusted in two and hanging under the car. i rigged it back together with rubber hose, but im sure it was the cause of my evap code.
 
Thanks scottydogs. U a mechanic?? Cuz u sure do know a **** ton. I'm gonna look right now. So it's connected to the tank and the top of filler neck.
