Gear indicator on shifter sometimes off, dash ok

I'm having an issue where sometimes I have got move the shifter around a few times so it correctly displays the gear at the shifter, though it's always correct on the dash, ie I can shift into drive and it can show neutral on console and drive on the dash, and I'm definitely not in neutral. Seems a little unusual since I'd think the indicator at the shifter should be a direct connection, as opposed to the dash which would need a cable and could get off. Any tips or suggestions before I dig into it?
 


Yesterday it stopped moving altogether - I just took it apart and found the issue. The little plastic piece that clips to the side of the shifter that the indicator rod slides through had fallen off (reinstalled it in the pic). On closer inspection it has a crack in it, epoxy is currently curing...
