Asking exactly what the title says...
Do you just remove the 4 bolts that attach the hinge to the door?

Just remove the 4 bolts and then there will be a wire harness connector that has to be unplugged and its off. On a 97 you have different style wiring connections than 99+ (I think) Anyways. If your swapping it out with a door from a different year save your old door harness.
The door I'm picking up is on a 97 I will be installing on a 98
Is it possible for the hinges to be removed from the body or are they welded... I believe mine are bad cuz my door sags a bit when it opens and catches the rear window molding when I shut it

My hinges are bolted on the door and on the car. So you can swap the hinges as well. If it's not aligned properly, it could just be how the hinges are aligned. There is some room to move the door up and down before tightening.
you can buy new hinges to fix your sagging door. no need for a whole door.
if you open the door and lift it from the outer most side, (outside door handle side) and the door lifts up, the hinges are bad.
