It doesn't appear in any of the engine diagrams I can find.

ive got a 5.3 in my tahoe, so not exactly the same set up, but my vac line from the top of the intake goes to a valve cover's pcv valve. its where you would install a catch can. between that line.
that other line looks fat enough to be evap related. im not even sure where im looking either in that pic.
Many LS engines have the PCV line into the valley cover and not the valve covers. But the LS4 should have a tube coming out of the rear valve cover that the PCV hose goes into.
The port on the intake manifold is likely another port used in a different applications. If you can put your finger over it and there is no air being sucked in, that port is blocked. My guess is that it's a PCV or EVAP line connection that just isn't used.
On my 08 that part is original and plastic, it goes to the port on the rear valve cover. The second picture that is a dummy post, nothing goes there and it has no vacuum.
You will have to contact a GM dealership directly for that.
The vehicle is over 10 years old and they don't have to make parts for it anymore. I don't see it listed on GM parts direct anymore so if there is a part number for that part, it's new old stock. They would have a complete parts list breakdown for it.
Or just do what anyone else would do and buy a piece of rubber hose to make a new one if you needed to do that.

As I said it's plastic on my car, the part I gave you is what is listed as it goes to the rear valve cover . If you have not owned the car since new chances are it's been replaced, possibly with a part from a truck or some other doner vehicle. It's not available so used or custom fit a new piece is your choice of repair.
