I managed to grab a '96-'99 N* off Ebay that came with the factory adapter for the 4.9. With some dremeling/epoxying/drilling/tapping, it bolted up and looks like it belongs there.
I spent ~$50 for the TB/adapter/shipping, and probably ~$10 on epoxy, brass fitting, RTV, hose, bolts.
I opted to use a 4" Ford MAF adapter from Spectre that I picked up for $10.
In total, I spent ~$70 for the TB, adapter, and all of the other pieces necessary to make this work.
The bottom two holes of the factory adapter piece will work, they just have to be elongated about .020". I have to drill some extra holes for the upper bolts to work (I've only done one of the upper holes, as of now). I removed the original 1/2" PCV pipe inlet, tapped the hole for 1/4" NPT, then added a 3/8" brass barbed fitting. I simply put a hose from the PCV fitting on the blower to the new brass fitting. Functional PCV, stock look!
I just need to get my hands on a pulley puller so I can get this blower on my car!
